The size is given in whatever unit the form used — millimetres, centimetres, inches or pixels — with the print resolution you need, so 35 by 45 millimetres at 300 DPI comes out as exactly 413 by 531 pixels. The background is replaced by flooding in from the edges of the frame, which means nothing inside the outline of the person is touched. Brightness and contrast are applied before the crop, because adjusting after a resize works on detail that has already been thrown away.
